The forms states "THIS MONEY NEVER HAS TO BE REPAID". It also states that CBS News and other sources reort that the U.S. Government must find recipients and distribute over $360 billion dollars to organizations and private individuals just like you.
At the bottom of the page the following name appears with a signature.
